Are you familiar with CHANT? Tennessee Department of Health recently streamlined three public health programsโ€”Help Us Grow Successfully (HUGS), Childrenโ€™s Special Services (CSS), and Tenncare Kids Community Outreach into ๐จ๐ง๐ž ๐ข๐ง๐ญ๐ž๐ ๐ซ๐š๐ญ๐ž๐ ๐ฆ๐จ๐๐ž๐ฅ ๐จ๐Ÿ ๐œ๐š๐ซ๐ž ๐œ๐จ๐จ๐ซ๐๐ข๐ง๐š๐ญ๐ข๐จ๐ง, the Community Health Access and Navigation in Tennessee (CHANT).
Those eligible to receive CHANT services include:
  • Children and Youth with Special Health Care Needs (Birth โ€“ 21 years)
  • Pregnant and postpartum adolescents and women
  • Children (Birth โ€“ 21 years)
