Tennessee Disability Services Study

We invite you to be part of a statewide survey focused on finding the best ways of connecting individuals with disabilities, families, and disability professionals with the information and services they need. Finding good information can be hard! Your feedback will help us improve the ways in which local resources are shared in Tennessee. We need your voice!

We want to hear from: 

  • Individuals with all types of disabilities 
  • Parents, siblings, spouses, and other family members of individuals with disabilities 
  • Educators, paraprofessionals, related services providers, and school leaders 
  • Direct support staff and providers who serve individuals with disabilities 
  • People working in disability organizations and state agencies focused on disability 

Everyone completing the survey can request free resources and be entered into a drawing for one of 100 gift cards ($20 each).  

This study is a collaboration among the Vanderbilt Kennedy Center UCEDD, Tennessee Disability Pathfinder, and the Tennessee Council on Developmental Disabilities.
