Health Care Transition and Youth
Health care transition can begin at any age but usually begins during the teenage years. The Center for Transition to Adult Health Care for Youth with Disabilities begins with young people and their ideas about health care transition. We want people with disabilities to live as independent and healthy lives as they choose.
